Welcome to the Holmes County General Health District Birth and Death Certificate online ordering system. Requests will be processed between Monday through Friday, 8:30 am to 4:00 pm.

GETTING A BIRTH OR DEATH CERTIFICATE

 

Birth Certificates 

  • The office only issues certificates for births that occurred in Ohio from 1906 – present. 
  • For births outside the State of Ohio, please contact the state in which the event occurred. 
  • For Birth records in Holmes county prior to 1906, contact the Holmes County Probate Court 330-674-5881.  For Birth records in any other county prior to 1906, contact the probate court of that county.

 

New Birth Certificates 

  • Certificates for new births typically takes up to 2 weeks to become available. While Holmes County General Health District Office of Vital Statistics strives to provide this service as quickly as possible, the information must first be received from the birthing hospital (or center) and uploaded into State of Ohio records system.
  • The Office of Vital Statistics processes orders within 2 business days once the information is available.

 

Death Certificates 

  • This office only issues certificates for deaths that occurred within the County limits of Holmes from 1906 – present.
  • The address of the death must fall within the County limits of Holmes.  If you are unsure if the address of the death falls within the County of Holmes you can use this site to confirm the address: Holmes Property Search  
  • For Death records in Holmes County prior to 1906, contact the Holmes County Probate Court 330-674-5881.  For Death records in any other county prior to 1906, contact the probate court of that county.
  • For deaths outside the State of Ohio, please contact the state in which the event occurred. 
  • For deaths occurring outside of the County of Holmes, but within Ohio, contact the county's office of vital statistics where the death occurred or the Ohio Department of Health.  
  • The year of death is mandatory to obtain a Death Certificate.  If you do not know the exact date please enter (99/99/2000).

 

Attention Applicants 

  • If you are applying for your own Birth Certificate and you have changed your name since birth, (Marriage, Legal Name Change, Adoption) pleae enter the reason for your name change in the 'Special Instructions' field on Step 4 of the application.

 

Social Security Numbers on Death Certificates
Per Ohio Revised Code (ORC) 3705.23(5): For the first five years after a decedent’s death, a decedent’s social security number shall not be included on a certified copy of the decedent’s death certificate unless that information is specifically requested to be on the certified copy. Relationship restrictions apply and the acceptable proof is required; for details, view the Social Security Number Guidelines and Addendum Form (PDF), effective October 15, 2015.
